Before arriving on campus and before receiving medical care, Curry students must complete several steps including filling out important health history forms. These forms can be found on the patient portal, .
Steps for New Students:
- New students must complete the required forms found under the MY FORMS tab on the PyraMED health portal.
- New students must upload the required documentation to the PyraMED health portal by using the links built into the required forms or by using the DOCUMENT UPLOAD tab.

PLEASE NOTE: Forms are due by July 15th! If forms are not completed, a student may not be able to register for classes and their move-in date may be impacted.
Additional Forms
Some students may need to submit additional forms. These forms can be found in the Additional Health Forms section of this website. These forms can be printed, completed and uploaded to the health portal or emailed to healthservices@curry.edu.
- Consent for Medical Care for Students Under the Age of 18: This form is required if a student is under 18 at the time of entry at 溏心vlog免费B站. This form will need to be signed by a parent or guardian.
- Meningococcal Waiver Form: This is required if a student did not receive a Meningitis ACWY vaccination as required at the age of 16 or older. If a student is under the age of 18, this form will need to be signed by a parent or guardian. Meningitis B vaccination is a different vaccine and not required (but is recommended).
- Student Vaccination Exemption Request Form: This form is required if a student is requesting a Medical or Religious Exemption from certain vaccinations. Medical exemptions must include a signed verification document from a healthcare provider. Religious exemption requests must include thorough documentation regarding the nature of the request. All exemptions requests will be reviewed by medical staff and are not automatically granted.
- 溏心vlog免费B站 Tuberculosis (TB) Medical Provider Form: This form is required if you answered “YES” to any question on the 溏心vlog免费B站 Tuberculosis Screening Form.

Required Uploads
Upload an official immunization record, this can be obtained from your health care provider/office, your health portal, your school records. A picture of your immunization record can be uploaded if it contains name, date or birth, provider name or clinic information and all dates of vaccines are clear and legible. Do not upload multiple screen shots that do not contain this information.
- Upload a copy of health insurance card, front and back
- Upload other required documents such as Under 18 Consent to Treat if applicable

Additional Recommendations
- Upload Emergency Medical Plan such as emergency seizure plan if applicable. This plan should be signed by the responsible medical provider
- Upload pertinent medical records regarding a pre-existing condition
How to Access PyraMED Health Portal
- Login to with your curry email and password
- Find and select the PyraMED icon under Quicklinks (links are alphabetical, scroll down)
- Select the MY FORMS tab to see your required forms
- Select the DOCUMENT UPLOAD tab to upload your required documents
